Application: The standardized form signed by a prospective insured that kick-starts an insurance policy in Canada. It asks for basic information (name, address, occupation, coverage desired), detailed risk questions (health history, property use, claims record, driving experience), and any additional statements or attached documents. The insurer reviews the answers—legally binding under common-law “utmost good faith” and CDIC/Quebec “duty to disclose”—to decide whether to issue, decline, or modify the policy. Whatever is written on or attached to the application becomes part of the final contract; undisclosed or misrepresented facts can allow the insurer to void or rescind the policy within the contestability periods set by provincial insurance acts.
